What is Smith Antibody?
The Smith (Sm) antibody test detects antibodies against Smith nuclear antigen, a marker highly specific for systemic lupus erythematosus.
Anti-Smith (anti-Sm) antibodies target small nuclear ribonucleoproteins and are considered one of the more specific autoantibodies for systemic lupus erythematosus (SLE), an autoimmune condition that can affect the skin, joints, kidneys, and other organs. This marker relates primarily to the immune system. Clinicians typically order Smith antibody testing as part of an ENA panel when lupus is suspected, especially after a positive ANA result, and particularly in people with symptoms such as joint pain, skin rashes, fatigue, or kidney-related changes. Because Smith antibodies are highly specific, though not present in every person with lupus, a positive result carries meaningful weight when combined with clinical features, while a negative result does not rule out lupus. As with all autoantibody testing, results are interpreted together with symptoms, physical findings, and additional laboratory tests.
