What is Calcium?
Calcium is a mineral essential for bones and teeth, muscle contraction, nerve signaling, and blood clotting. The small fraction circulating in blood is tightly regulated by hormones.
Although about 99% of the body's calcium is stored in bone, the calcium measured in blood is what supports moment-to-moment functions like heartbeat, muscle contraction, and clotting, and the body defends this blood level closely using parathyroid hormone and vitamin D. A standard blood test reports total calcium, which includes calcium bound to proteins such as albumin, so a low albumin can lower the total reading without truly lowering the active (ionized) calcium. Persistently high blood calcium (hypercalcemia) can stem from overactive parathyroid glands or other conditions, while low calcium (hypocalcemia) can accompany vitamin D deficiency, kidney disease, or parathyroid problems. Because of the albumin effect, clinicians sometimes adjust the value or measure ionized calcium directly.

How to read a value in context
A normal total calcium means the value is within the lab's reference interval, but because much of blood calcium is bound to albumin, a normal or abnormal total can be misleading when albumin is off. The tightly regulated active fraction is what the body controls, so clinicians may consider albumin or measure ionized calcium when the picture is unclear.
