Aspartate aminotransferase (AST) is an enzyme present in the liver but also in the heart, skeletal muscle, kidneys, and red blood cells, so an elevated AST is less liver-specific than ALT and is usually interpreted together with it. Damaged cells release AST into the blood, which is why AST is included in standard liver function panels and compared against ALT to describe the overall pattern. Because AST comes from several tissues, factors such as vigorous exercise, muscle injury, or heart events can raise it independently of the liver. The absolute AST value does not measure the extent of any organ injury, and results can be affected by age, sex, and how the blood sample was handled. A clinician interprets AST in context, often using the AST-to-ALT relationship as one clue among many.

A 'normal' AST means the result falls within the population-based reference interval the lab established for its assay, not that liver or muscle tissue is certified healthy. AST is most useful when trended and read alongside ALT and the rest of the panel.
